1. Am I required to report a dog bite to Animal Control?

Yes, all bites are required by law to be reported to Animal Control or the Department of Health. Dogs or cats not current on their Rabies vaccinations are also required by law to be quarantined for a period of 10 days at the Animal Shelter. If your animal is current on their rabies vaccination the animal may be quarantined at your home in an approved confined setting. Contact the Animal Shelter at 336-903-7688.

2. How long do you keep the animals?

We hold stray dogs and cats for 72 hours (3 days) to see if anyone will claim them. There is no set time for the animals that are up for adoption. That time greatly depends on how much kennel space we have from day to day which is frequently limited.

1. What is diabetes?

Diabetes means that your blood glucose (blood sugar) is too high. There are several types of diabetes.
- Type 1 diabetes: the body does not make insulin. Insulin helps the body use glucose from food for energy. People with type 1 need to take insulin every day.
- Type 2 diabetes: the body does not make or use insulin well. People with type 2 often need to take pills or insulin. Type 2 is the most common form of diabetes.
- Gestational diabetes: may occur when a woman is pregnant. Gestational diabetes raises her risk of getting another type of diabetes, mostly type 2, for the rest of her life. It also raises her child’s risk of being overweight and getting diabetes.

2. What is a Registered Dietitian?

A registered dietitian is a food and nutrition expert who has met academic and professional requirements including:
- Earned a bachelor’s degree with course work approved by the Academy of Nutrition and Dietetics’ Accreditation Council for Education in Nutrition and Dietetics (ACEND). Coursework typically includes food and nutrition sciences, food service systems management, business, economics, computer science, sociology, biochemistry, physiology, microbiology and chemistry.
- Completed an accredited, supervised practice program at a health-care facility, community agency, or food service corporation.
- Passed a national examination administered by the Commission on Dietetic Registration.
- Completes continuing professional educational requirements to maintain registration

3. What is a Certified Diabetes Educator?

A Certified Diabetes Educator® (CDE®) is a medical/health care professional who possesses comprehensive knowledge of and experience in diabetes management, pre-diabetes, and diabetes prevention. A CDE® educates and supports people affected by diabetes to understand and manage the condition. A CDE® promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes.

4. What is Diabetes Self-Management Education?

Diabetes self-management education (DSME) is an interactive, collaborative process that can equip adults with basic knowledge to manage their type 2 diabetes while focusing on their self-identified problems and goals. It emphasizes problem solving and decision making as they relate to core diabetes self-care skills such as healthy eating, physical activity, proper dental care, and monitoring blood glucose level.

The goals of DSME are to improve metabolic control and quality of life, to reduce diabetes-related complications, and to minimize health care costs. Research has shown that community-based DSME is an effective intervention for improving glycemic control among adults of various racial and ethnic backgrounds with type 2 diabetes.

Elections

1. I think I am registered to vote, but I am not sure, how do I find out?

You can call the Wilkes County Board of Elections office at 336-651-7339 or you can check on the North Carolina State Board of Elections website.

2. Where can I vote if I can not vote at my precinct on Election Day?

One-Stop voting begins on the third Thursday prior to the date of an election and ends on the Saturday before the Election. In even numbered years, the One-Stop voting location for Wilkes County is in the County Commissioner's Meeting Room on the first floor of the Wilkes County Office Building for the Primary and General Election. Check the Absentee/One Stop tab for all dates and locations.

3. How can I request an Absentee Ballot by mail?

To apply for an absentee ballot by mail, send a NC State Absentee Ballot Request Form to

The form can be mailed, emailed, faxed or brought in person to our office by the voter, near relative or verifiable legal guardian.

Upon receipt of your request, the county board of elections will send you an absentee ballot and instructions for completing the ballot. You must submit your request no later than the Tuesday prior to Election Day. The request may be made by the voter, a near relative or verifiable legal guardian. 4. Can I vote by Absentee ballot?

If you can not vote in person, absentee voting is a way to vote. There are two ways of voting before the election:
- One-stop and mail out absentee.
- Check the Absentee/One Stop tab for more information.

5. What if I have disabilities that make my precinct not accessible to me?

Counties are trying hard to improve accessibility. You may vote from your vehicle at your precinct. This is called curbside voting, if you park in the Curbside space someone will assist you. You can also vote at the Wilkes County Board of Elections Office Building during One-Stop voting. The County Office Building is located at 110 North Street. During even numbered election years, One-Stop will be held on the first floor of the building in the County Commissioner's Meeting Room.

6. Where do I vote?

Your voter card lists the polling place where you vote. Each voter is sent a card when they register to vote or when the voter makes a change to their voter information. If you do not know where the polling place is call the Wilkes County Board of Elections at 336-651-7339 or you may look up your polling place at NCSBE voter lookup.

7. If I registered by mail and have not voted before, do I have to do anything different?

If you did not include your North Carolina Drivers Number or the last four digits of your Social Security Number when you registered to vote, you will be required to show identification the first time you go to the polls to vote. (If you completed this with your registration form, then it is not required).

8. I do not own property, so why does it matter whether or not I vote?

Everyone who pays taxes should vote. Even if you do not own property, you pay taxes on vehicles and any items that you purchase. Every person over the age of 18 should be involved in electing individuals who control the spending of your tax dollars.

9. Will registering to vote increase my chances of being called for Jury Duty?

No. Anyone who has a driver's license issued in North Carolina is eligible to be called for Jury Duty.

10. If I registered before the election last year do I need to register again?

No. Wilkes has free, permanent and continuous registration. You need to register only if:
- You have not registered in Wilkes County before
- You have moved
- You wish to change your party affiliation
- Your name has changed
- You have not kept your address current, and have not voted in two federal elections

11. How can I register to vote?

You can register at the Board of Elections office located at 110 North St. Wilkesboro. The office is open from 8:30 a.m. until 5:00 p.m. Monday thru Friday. You can also register at the DMV if you are renewing your driver’s license. DMV customers may register to vote through the DMV website, a DMV transactions is not required.  The Employment Security Commission, Wilkes Public Library, and certain other public agencies also have the forms available. The form is available under the Voter Information tab. The form must be printed, filled out then mailed or returned in person to our office. A registration form must be postmarked or delivered to the Board of Elections office no later that 25 days before an election in order to be valid for that election.

12. What are the requirements to be able to register to vote?

- You must be a citizen of the United States(if convicted of a felony, citizenship rights must have been restored)
- A person 18 years of age or older
- A legal resident of Wilkes County for 30 days by the date of the next election

13. When are the polls open on Election Day?

All polling places at 6:30 am and close at 7:30 pm.

14. Can I vote in a precinct where I don't live?

Voters who appear on Election Day in the correct county but in the incorrect precinct may cast a provisional ballot, which will be counted for all contests in which the voter was eligible to participate. 

15. I am registered Unaffiliated, may I vote in a Primary Election?

You are eligible, based on current laws, to vote in either party primary (only those who are open to Unaffiliated voters) or to choose a Non-Partisan ballot. You must declare which ballot you desire when you go to vote. This does not change your registered party.

16. My relative has passed away recently. How can I request their name to be removed from the voter rolls?

Pursuant to NCGS 163-82.14(b), the only time a person is authorized to cancel the registration of another person is when the voter is near relative cancelling the registration of a deceased family member. The Deceased Voter Removal Form must be completed by a near relative and returned to the Wilkes Board of Elections to remove your relative from our voter registration records. Deceased Voter Removal Form

1. Who do I contact concerning invoices submitted for payment?

If you have a question about the status of a payment, please contact the county department you provided the product or service for. Each department is responsible for sending the invoices to us for payment in a timely manner. View the Staff Directory.

2. How often do you write accounts payable checks? When can I expect to receive payment for an invoice?

1. Do you accept walk-ins?

2. How can I get dental care?

You can contact the Wilkes Public Health Dental Clinic at 903-9399 or WYO in Yadkin County at 462-2073 both of these clinics see self-pay patients.

3. How do I download a Sports Form?

If you are looking for sports physical forms or information concerning sports physicals and Mobile Expanded School Health (MESH) services, please visit the MESH page.

4. What is a sports physical?

A sports physical is a physical exam completed by a doctor or nurse practitioner that enables the student/child to be able to play sports. The required sports physical form from the North Carolina Athletics Association is completed.

5. I am a temporary resident of North Carolina. How can I get a refill on a prescription without an appointment?

Here are a couple of suggestions:
- Contact the doctor that prescribed your medication and see if they will call a refill into a North Carolina pharmacy.
- Since you are going to be in North Carolina temporarily, see if an urgent care would refill this medication for you.

6. I am concerned about a pregnant mother using illegal drugs. What do I do?

Contact DSS, at 336-651-7400, if there is a concern regarding drug abuse and a small child in the home. There is nothing legally that can be done while a person is pregnant in North Carolina. DSS can flag a chart and follow-up after the infant is born but cannot do anything while the person is pregnant.

7. Are there any free services for Alzheimer’s patients and/or their care-givers?

Depending on the type of services that you looking for, the Alzheimer’s Association has numerous resources. You might also contact a social worker at any of the three skilled nursing facilities (nursing homes) in the county; Wilkes Senior Village, Westwood Hills, or Avante. The social worker might have additional resources they could share.

9. How do you refer someone to your services? And is this Health Department for profit or non profit?

10. Why do I always experience chest pain, my neck tightening, wheezing, fatigue, and difficulty breathing when I smell cigarette smoke?

This could be an allergic reaction to cigarette smoke. You should avoid being in contact with cigarette smoke.

11. Are electronic cigarettes safe to use?

Electronic cigarettes are not regulated, although the Food and Drug Administration (FDA) is considering regulations now. Without any type of regulation, it is hard to know exactly what is in the e-liquids used in these products. Manufacture of e-liquids can be done without providing consumers information about what is actually in the liquid. E-cigs are a relatively new product, so there are not many studies that provide information on the effects of vapor on health. E-cigs still contain the addictive component of nicotine which, has its own set of ill health effects, including heart disease and hampered brain development in children and adolescents.

“ENDS” (Electronic Nicotine Delivery System), is a name that refers to all electronic cigarettes (e-cigs) and any other device that may be considered a “vapor product.”

“Since ENDS are not yet regulated as tobacco products under the Tobacco Control Act, we have very little information about the ingredients of liquids (purity, impurities or stability), or the approximate exposure to harmful and potentially harmful constituents when using electronic cigarettes over the short-term or long-term. To date, manufacturers are not required to publish what chemicals are in the ENDS solution, or to perform or reveal results from systematic testing. Studies have demonstrated wide variability in design, operation, and contents and emissions of carcinogens, other toxicants, and nicotine from ENDS. Depending on the brand, ENDS cartridges typically contain nicotine, a component to produce the aerosol (e.g., propylene glycol or glycerol), and flavorings (e.g., fruit, mint, or chocolate). Potentially harmful constituents also have been documented in some ENDS, including: irritants, toxins that can change genes, and other ingredients that have been shown to cause cancer in animals.”

14. Is it ok to bake and sell cupcakes from my home or do I need to use an industrial kitchen?

To bake and sell cupcakes in your home you need approval from the North Carolina Department of Agriculture. They may be reached at 919-733-7366

15. Are “shot bloks” good to eat when sick and low on energy?

I would not recommend the shot bloks from Cliffbar for sickness and low energy alone. The bloks are meant to be used for and “instant” fuel source to the muscles during time of long term (greater than 1 hour) moderate to vigorous physical activity. For times of sickness and low energy I would focus on maintaining adequate fluids through water first and then if necessary a beverage to replace electrolytes. Also focusing on fruits as a source of food and fluid.

16. How long does a stomach bug last?

It varies depending on what is causing the upset stomach. It can last from 24 hours up to several days. You should contact your doctor for guidance on how to manage the stomach bug since he/she knows your medical history and can make the best recommendation for your care. If you do not have a medical doctor and are uninsured, contact Armando Limon at 336-651-7386 for assistance with finding a medical doctor.

17. Can a person be seen in your dental clinic, if they live in another county?

18. Does Health Department help individuals with eye doctor vouchers?

We do not provide eye exam vouchers, but we can help with an eye exam through our Health Care Connection program, if you qualify for the program. Please call the coordinator of the program, Armando Limon at 336-651-7386.

19. Who qualifies for placement in a nursing home/assisted living facility?

Placement in a nursing home or another facility begins with a conversation with the physician of the person seeking placement. Speaking with the physician and letting them know your concerns and talking about options is the best place to start. When selecting a facility, such as a nursing home, assisted living facility, etc, it is always best to tour the facility. It is recommended to do a “drop by visit” and see how the facility operates before scheduling the tour.

It is also recommended to view the facility’s last state survey results. The facility will also have the results readily available upon request. If you have any questions with the survey results have the administrator review it with you. For more information regarding assisted living facilities or skilled nursing facilities please visit the North Carolina Divison of Health Service website.

20. How do I get a birth certificate?

You will have to contact the Wilkes County Register of Deeds.

21. Does the Health Department keep a full death certificate for individuals? How do I get a copy?

The Health Department does not keep the full death certificate of an individual. All death certificates and birth certificates are sent to vital records in Raleigh. If you need a copy please contact the Register of Deeds at 336-651-7351, contact the Health Department at 336-651-7450, or visit the North Carolina Vital Records website.

22. Can someone from another county sign a death certificate?

A death certificate is signed by a the Medical Examiner, a MD, nurse practitioner, or physician assistant from the county in which the person died. So for example, a person who lives in Wilkes County dies in Forsyth County, then the Medical Examiner, a MD, nurse practitioner or physician assistant from Forsyth County would sign their death certificate.

23. What do you do if you have a Urinary Tract Infection (UTI)?

If you think you might have an UTI, then an appointment needs to be scheduled as soon as possible with your doctor. A UTI could potentially be a dangerous diagnosis, sepsis can possibly be developed if left untreated. If diagnosed with a UTI, then your doctor will take the proper course of treatment, this is usually prescribing an antibiotic.

1. Who do I contact if I am concerned about environmental health hazards in my work place?

We recommend that you contact the Department of Labor’s Occupational Safety and Health Standards and Interpretations office at 800-625-2267 or visit the Department of Labor's website.

2. Who can I speak to concerning black mold in a rental house?

Contact the North Carolina Department of Health and Human Services Occupational and Environmental Epidemiology at 919-707-5900. Mold kits can be purchased from Lowe’s Hardware. You can also contact a Mold Inspector by the name of Mike Cole at 828-326-9100 to come out and complete an inspection. There is a charge for this service and does not involve the health department.

3. How do I report improper removals of waste in my neighborhood?

Please contact the Department of Health and Human Services office of Occupational and Environmental Epidemiology at 919-707-5900 or contact the North Carolina Division of Water Resources at 919-707-9000.

4. How can I get help with code violations in my apartment?

Wilkes County residents can contact the Planning and Zoning Department at 336-651-7582 and Building Inspections Department at 336-651-7320.

5. Have there been reports of bed bugs at any lodging establishments in Wilkes County?

Bed bugs are getting more common in all lodging establishments. For more information please call 336-651-7530.

6. Is it illegal to have a camper on your own land and allow the raw sewage to be dumped on the owners property?

In accordance with NCGS 130A-335 (h), It is unlawful to discharge sewage or other waste from a chemical or portable toilet used for human waste except into an approved wastewater system. Feel free to contact the Environmental Health office at 336-651-7530 if you have any further questions.

7. Does health department do drinking water testing for individuals?

Yes. If an individual is interested in having their water tested, an application must be submitted to our office. Depending on the type of water sample requested the fees are typically $125.00 per test. The water sample is collected and sent to the State Lab for analysis it typically takes 3 to 4 weeks to receive the results. 8. Does the Health Department keep records of applications for new septic systems?

Yes. The new septic system application is attached or filed accordingly to the type of service the new septic system application receives such as permitted, refunded, revoked or suspended. If you have any questions, please contact our Environmental Health division at 336-651-7530.

9. How can I report illegal activities/poor sanitation in hotels?

To file a complaint, please call our Environmental Health Department at 336-651-7530.

10. How can we have someone from the Health Department check out unsanitary living conditions?

For issues with trash and rodents, contact Andrea VanWy with the Landfill at 336-696-5832. If there is a neglect issue for children, contact Social Service 336-651-7400. For animal neglect and related issues such as rabies, contact Animal Control at 336-903-7688.

For issues with private residence and rented property related to housing conditions, personal legal assistance must be obtained. The Wilkes County Health Department only has jurisdiction over facilities that require inspections. If further assistance is needed please contact our office at 336-651-7530.

11. Are there health concerns or city ordinances for trash piles in a neighborhood?

Yes, there is a County Solid Waste Ordinance that is in place for such nuisances. Please contact Anderia Byrd at 336-696-5832 during regular business hours and be able to provide violator information, such as address at which said solid waste is located and if possible owner information.

12. Are there records in the Health Department that indicate where the septic tank and drain lines are located in old houses?

Environmental Health didn’t start keeping records until around 1973 and then it was 1975-1976 before most people started registering their septic systems with the Health Department. It depends on when the house was built and then I would need a parcel number, original owner’s name and possible more information to try and locate these records. 13. How long is a perc test valid?

If a septic permit has been issued, the permit is good for 5 years. One or more of the following could invalidate the permit:
- The original site plan has changed (house location, drive, number of bedrooms, etc.)
- The site has been disturbed, graded, etc.
- Property dimensions have changed
- Other factors based on the permit conditions

14. What is the cost of a Perc Test?

Perc test fees run by the number of bedrooms. Each bedroom is $120 so a three bedroom system would be $360. The smallest system we do for dwellings (homes) is 2 bedrooms. For more information please visit the Fees page.

15. What do I do if there is lead or asbestos in a home?

Thank you for your concerns regarding the health impacts of asbestos and lead in a home. In Wilkes County, we currently don’t have a program regulating the demolition or removal of asbestos or lead paint through the Wilkes County Health Department. However, I can provide you with a contact telephone number for a branch that regulate asbestos demolition, removal and health related issues. This department is located out of Raleigh with a contact number of 919-707-5950.

Lead paint in a home can be hazards to children. Lead base paint can typically be found in home that were built before 1978. At the time it was the good paint, but has since been linked to children with elevated blood lead levels. Children typically have their lead level evaluated by their pediatrics at the age of 2 and 6. If it is determined that the child has an evaluated level then the pediatric refers them to the Wilkes County Health Department.

A team investigation between the Lead Nurse, Environmental Health and the family is conducted. We investigate the home, foods, toys and activities to determine and eliminate the source. If we can help with additional concerns, please feel free to contact us at 336-651-7530.

Health Department - Family Planning

1. Are there any drugs that I can take to prevent me from getting pregnant?

Emergency Contraception is available for up to 72 hours after unprotected sex. If you plan to remain sexually active, please contact Wilkes County Health Department to schedule an appointment in our Family Planning Clinic to prevent an unplanned pregnancy.

2. Can teens receive birth control without their parent's knowledge?

When insurance is billed, the insurance company sends a statement to the individual that has the insurance. If you are a teenager, you may be seen confidentially in our Family Planning Clinic. If you don’t have insurance or adequate income to pay for birth control pills, they may be provided at no cost to you. For specific questions and to schedule an appointment, call 336-651-7450.

3. Can you get pregnant during or right after your period?

Yes, you can. So, if you’re not actively trying to get pregnant, it’s always safer to use reliable contraception when you have sex, no matter when it is during your cycle. Ovulation occurs around the middle of a woman’s cycle – specifically, in the middle third. If you have a 28-day cycle, for example, then you usually ovulate on the 14th day, counting the first day of your period as the first day of your cycle.

If your period lasts seven or eight days, by the time you finish menstruating you’re only five or six days away from ovulating. If you consider that sperm can live in the fallopian tube for two or three days, then you have a narrow window in which you could get pregnant if you ovulated a little earlier than normal.

4. Does the Health Clinic provide abortions?

No.

5. How effective are oral contraceptives?

The pill does not protect against sexually transmitted diseases (STDs), including HIV (the virus that causes AIDS). The latex male condom provides the best protection from most STDs. For information on how effective birth control methods are, visit the Center for Disease Control and Prevention website.

6. Is the birth control pill free of charge without insurance? Do you need a pelvic exam/ Pap smear for get prescribed for the pill?

If you are a patient in the Family Planning Clinic and don’t have insurance, your birth control pills may be provided at no cost to you depending on your income. For specific questions and to schedule an appointment, call 336-651-7450.

7. I think I might be pregnant. What should I do?

Either complete a home pregnancy test or schedule an appointment with Wilkes County Health Department for a pregnancy test. If you plan to remain sexually active and to prevent an unplanned pregnancy, schedule an appointment to be seen in our Family Planning Clinic. The number to call is 336-651-7450.

8. Is the pregnancy test free?

There is a charge of $15 for a pregnancy test. If you would like to schedule an appointment for a pregnancy test please call 336-651-4550.

9. I had unprotected sexual intercourse 6 days ago. Are there any drugs that I can take to prevent me from getting pregnant?

Emergency contraception is only effective up to 3 days after unprotected sex. If you think you might be pregnant please contact your primary care physician or call our office at 336-651-7450 to set up an appointment with our family planning nurse.

10. Has the state of North Carolina passed a law regarding babies born with drug dependency?

For any concerns that you may have about the well-being of a child, please contact Department of Social Services at 336-651-7400.

11. Does the Health Department do paternity testing?

No, the Health Department does not provide paternity testing.

12. How much is the depo birth control shot?

This shot would be free of charge. If you have any other questions, please feel free to call us at 336-651-7450.

13. How do you get herpes?

You can get herpes by having vaginal, anal, or oral sex with someone who has the disease. Herpes is spread through skin to skin contact. Fluids found in a herpes sore carry the virus, and contact with those fluids can cause infection.

You can also get herpes from an infected sex partner who does not have a visible sore or who may not know he or she is infected because the virus can be released through your skin and spread the infection to your sex partner.The only way to avoid Sexually Transmitted Diseases (STDs) is to not have vaginal, anal, or oral sex. If you are sexually active, you can lower your chances of getting herpes by being in a long-term monogamous relationship with someone who has been tested for STDs and has tested negative for STDs, and by using latex condoms the right way every time you have sex. Herpes symptoms can occur in both male and female genital areas that are covered by a latex condom. However, outbreaks can also occur in areas that are not covered by a condom so condoms may not fully protect you from getting herpes.

Most people who have herpes have no, or very mild symptoms. You may not notice mild symptoms or you may mistake them for another skin condition, such as a pimple or ingrown hair. You should be examined by your doctor if you notice any of these symptoms or if your partner has an STD or symptoms of an STD, such as an unusual

14. My period is abnormal. What do I do?

If you have any symptoms such as increased pain or new problems with your period, please schedule an appointment to be seen with your family doctor. If you are sexually active, you may need to complete a home pregnancy test or have a pregnancy test at the Health Department.

15. What do you do if you want to have an abortion? Is there an abortion pill offered?

19. If you’re under age 18 can you get free birth control?

You should call 336-651-7450 and make an appointment for a Family Planning Enrollment visit. At that visit the provider will discuss birth control options with you and decide which method is best for you. A parent does not have to accompany you but we do encourage parental involvement.

20. If you are pregnant without knowing and you continue to take your prescription birth control, will you lose your baby?

Increased hormone levels from the birth control pill could cause birth defects or loss of pregnancy. If you have a missed a period, it is a good idea to take a pregnancy test to ensure that you are or are not pregnant.

21. What is your Maternal Nurse and what does she do?

We have Pregnancy Care Managers who are either Social Workers or Nurses. The Pregnancy Care Managers work with the patient’s OB doctor to help manage the care of the patient while she is pregnant. Their job is to assist the patient during the pregnancy to meet the patient’s needs and goals. These staff can help with community resources if needed. The ultimate goal is to have a healthy pregnancy and a healthy birth delivery.

The Postpartum/Newborn nurse is a part of the Welcome Baby Program through the Health Department. It is a free service offered to all new mothers and babies. What the Postpartum/Newborn nurse does is schedule a one time home visit with the new mother and child. They assess both mother and baby, provide education, and detect possible problems early. They also provide education about normal newborn characteristics, feeding issues, and community resources as needed. Certified North Carolina Lactation Educators can assist mother with breastfeeding and any issues that may be occurring.

22. I am pregnant and under the age of 18. Should I apply for Medicaid now or wait until I am 18?

You can either go now or wait until you are 18 years old, it does not matter, to sign up for Medicaid. The type of Medicaid that you would receive does not end until you are 19 years old. Medicaid will cover you through your pregnancy. However you must go to the Department of Social Services (DSS) to sign up for Medicaid. DSS will want proof of your pregnancy, so you can get that from the Crisis Center, and take it with you when you go to apply for Medicaid. For more information, please contact DSS at 336-651-7400.

23. Are the condoms you give as good as the name brand condoms like Trojan?

Yes. The brand of condoms that we provide here at the Health Department, Lifestyles, are a name brand and as good as any other brand that you can purchase at a pharmacy. Condom manufacturers quality test their product to make sure they meet industry standards before they arrive on the shelf. In addition to the manufactures testing, the U.S. Food and Drug Administration performs routine Good Manufacturing Practice (GMP) inspections.

24. How long should I wait to take a pregnancy test?

In order to be tested at the Wilkes County Health Department, a woman’s menstrual period must be at least seven days overdue/late. Some tests will pick up the pregnancy a few days before a period is due, but it’s always best to wait until a menses is missed.

25. Do you offer the Plan B pill?

Yes. Call the Health Department at 336-651-7450 to make an appointment for emergency contraception. With emergency contraception, you must take the pill within 72 hours (3 days) of failure of your regular form of birth control or after having unprotected sex.

26. Is there any help offered to uninsured women who are due for a mammogram?

There are two programs that the Health Department participates in that could offer assistance if you need a mammogram, the Breast and Cervical Cancer Control Program (BCCCP) and the Susan G. Komen Race for the Cure NC Triad.

27. Does the Health Department offer the abortion pill?

The Morning-After Pill or Plan B Pill is often referred to as the abortion pill. The Morning-After Pill or Plan B Pill is not the same as RU-486, which is an abortion pill. It does not cause a miscarriage or abortion. In other words, it does not stop development of a fetus once the fertilized egg implants in your uterus. So it will not work if you are already pregnant when you take it.

The Health Department does provide the Morning-After Pill or Plan B Pill. Call the Health Department at 336-651-7450 to make an appointment for emergency contraception. With emergency contraception, you must take the pill within 72 hours (3 days) of failure of your regular form of birth control or after having unprotected sex.

28. Can you have your period or bleed while you are pregnant?

Uterine bleeding somewhat suggestive of the menstrual period occurs occasionally after conception. One or two episodes of bloody discharge, somewhat suggestive of and sometimes interpreted as menstruation, are not uncommon during the first half of a pregnancy. Almost always such bleeding is short and scant. If at anytime during your pregnancy you have any questions or concerns to be sure that you contact your OB/GYN.

7. Will Ohio Medicaid pay for Genome Mapping for adopted children with Medicaid who have no family medical history/background?

In North Carolina, we would refer the recipient to the provider of the service (the agency doing the testing) if they are able to bill Medicaid for the service provided. Each state sets its own rules for what Medicaid will pay. Another option may be to contact the adoption agency to see if there are adoption assistance monies available for the testing. 8. Is there a charge for Mobile Expanded School Health (MESH) visits? If a child without insurance or Medicaid gets sick at school could they be seen?

MESH does not charge for services. If a student has Medicaid or Health Choice then we can bill the insurance. The only fee we charge is for a sports physical which is $15. There is no co-pay or fee for sick visits. If we know who the doctor is then MESH will fax them a copy of the sick visit to the doctor for their records.

3. Do you offer services for eating disorders?

You need to schedule an appointment to see your family doctor to discuss this issue. He/she will discuss your concerns with you and may decide that blood work is needed also.

Contact the Mobile Expanded School Health (MESH) Unit staff that comes to your high school to schedule an appointment to discuss this with them. If the provider determines that you need counseling, she will refer you to a counselor that works with high school students.

12. Is there a tuberculosis vaccination?

Currently there is no vaccination for prevention of tuberculosis (TB). People who have been exposed to someone with active tuberculosis may become infected, and not be actively sick, nor are they contagious. Those individuals are encouraged to take antibiotics to reduce the likelihood that they will develop active tuberculosis later, when circumstances may reduce their immune system’s ability to contain the infection. Those efforts are helping us to see a reduction in the number of cases of active TB in North Carolina. However, TB continues to be extremely contagious and requires public awareness to reduce its spread.

8. What if I do not have a social security number and am not eligible for one?

You must complete the Affidavit of Ineligibility and have it properly notarized. You may then present this form and proceed with the marriage license application. You may obtain this form either from our office or download from the forms section of this website.

9. How do I change my name and/or address?

North Carolina General Statute 10A-13(b) states that within 30 days after changing names, a notary shall notify the Secretary of State of the change by submitting a name/address change form. If the county changes due to the address change, the notary may continue to use the seal or stamp for the previous county of commission and at the time of expiration re-apply for the county of residence.

There is no fee to the Secretary of State for these changes. Once you receive notice from the Secretary of State that the change has been processed, you must come to Wilkes County Register of Deeds office to be sworn in. 1. How can I get my deed of trust canceled of record?

Typically there are three ways to cancel a deed of trust of record.
- Cancellation by Exhibition: The presentation to the Register of Deeds of both the original deed of trust and the original promissory note. Both must be endorsed by any officer of the lender (beneficiary) with the name of the lender and the date of satisfaction. The signature portion of the endorsement of payment and satisfaction must be followed by the printed, typed or stamped name of the endorser and their capacity (title).
- Certificate of Satisfaction: This is used when either the original deed of trust or both deed of trust and promissory note are missing. This is executed by the lender (beneficiary) and takes the place of the original deed of trust. If the original note is available, the note must be marked paid and satisfied as discussed in cancellation by exhibition. The date of satisfaction recited in the certificate of satisfaction must be the same as the date of satisfaction on the promissory note. The certificate of satisfaction must be properly signed and notarized. If the original promissory note is lost, an affidavit of lost note must accompany the certificate of satisfaction.
- Notice of Satisfaction: This is executed by the trustee or the substitute trustee. The notice of satisfaction must be properly notarized.

There is no fee for processing a cancellation.

2. What are the requirements for recordation of a deed or deed of trust?

The name of the draftsman must appear on the first page of the document. This must be either a law firm or an individual's name. This should be prefaced with the words "Prepared by" or Drawn by". In addition, the first page should contain the name of the person to whom the instrument is to be returned. This should be prefaced with the words "Return to".

If you are recording a gift deed, this must be stated on the first page. If the deed is not a gift, you must declare and pay excise tax consideration. The excise tax must be noted on the first page of the deed. Instruments must be properly signed and notarized.

3. What is the recording fee of a deed of trust?

The filing fee is $56 for the first 15 pages plus $4 for each additional page.

5. How do I calculate excise tax?

The calculation of excise tax is $1 per each $500 or a fractional part thereof of the purchase price or value of the property if a trade is negotiated.

6. What are the specific criteria of the new document standards?

The recording standards provided for in G.S. 161-14(b) are effective for all instruments executed on or after October 1, 2011. After this date, any instrument presented for registration must meet the following requirements:
- Presented on 8.5 by 11 inch or 8.5 by 14 inch paper
- Has a blank margin at top of first page of 3 inches and one quarter inch on remaining sides of the first page and all sides of subsequent pages
- Be typed or printed in black on white paper in a legible font that is not smaller than 9 points in size
- Is printed in single-sided pages
- Indicates the type of instrument at the top of the first page

The new requirements permit blanks to be filled in and corrections to be made by hand in pen. If a document executed on or after October 1, 2011 does not meet the new standards, the document can still be recorded. However, the Register of Deeds will collect a new $25 fee for filing a "nonstandard document." This fee will be in addition to the filing fee for the actual recording. The National Standard Form for Uniform Commercial Code (UCC) will be excepted from the "nonstandard document" fee.

1. When was the Uniform Commercial Code revised?

The Uniform Commercial Code Article 9 was revised effective July 1, 2001. The major changes are that only the financial statements that will be filed in the local Register of Deeds office are those in which the collateral is as-extracted collateral, timber to be cut, or fixtures. In addition, the filing fees have increased.

2. What types of UCCs are filed at the local Register of Deeds office as a result of Revised Article 9?

Financing statements filed locally are an initial financing statement for fixture filings (related to real property) and an amendment (continuation, assignment, termination) of that same record. If a fixture filing financing statement was filed at the local Register of Deeds office prior to July 1, 2001, a termination of that record will also be filed locally. All other financing statements will be filed at the Secretary of State.

3. What are the new filing fees for UCCs?

Filing fees for initial financing statements, correction statements, and all amendments, including terminations are as follows:
- One or Two Pages $38
- Three to ten pages $45
- Over ten pages $45 plus $2 per page over ten

Fees for providing information (UCC-11) on UCCs filed before July 1, 2001:
- $30 per name searched
- $1 per copy provided

1. How many children can foster parents have in their home?

Foster homes can have up to five children in their home. This includes their own children.

2. Do foster children require a room to themselves?

No, foster children can share a bedroom with other children in the home.

3. What age children are in foster care?

Children of all ages enter Foster Care. Children as young as infants up to the age of 18 may need a foster home placement. Young adults ages 18-21 can also be eligible, based on requirements, to participate in the Foster Care Program. Potential  foster parents will go through a training process that will help them identify what age group they are able to foster in their home. The Department of Social Services is always seeking more foster families, especially those that will consider fostering teenagers and sibling groups.
