Eligibility
Care Coordination for Children (CC4C) is a program offered at no charge for children birth to 5 years of age who:
- Have long term medical conditions
- Are dealing with social issues
- Are referred by the child's doctor
- Have a recent emergency department or hospital visit
When a CC4C care manger learns that your child qualifies for services, they will contact you. The care manager will help you determine if CC4C services are good for your child and family. If so, a CC4C plan of care is developed.
Care Managers
Care managers help coordinate health care services by making home visits, phone calls and can even visit the doctor with you. Care managers recognize that you are the expert on your child. In addition, care mangers:
- Will help you build a strong link to a medical home (regular doctor) for your child
- Offer referrals to community resources to meet your family's specific needs
- Offer information on health and family topics
- Help you get answers to your questions, or concerns
Health Insurance
Your child may qualify for Health Check (Medicaid) or North Carolina Health Choice-free or low cost health insurance for children and teens under 19 years old (up to 21 for Medicaid). Even children in a family of four with working parents earning $51,276 a year or slightly more may qualify. The programs also cover children living with grandparents, other family or friends. Your children need a valid social security number to qualify for these programs. Families with children who are U.S. citizens or lawfully residing in the U.S. are encouraged to apply.
